Can consuming caffeine while breastfeeding harm your baby?

New Rochelle, NY, February 21, 2012–Babies are not able to metabolize or excrete caffeine very well, so a breastfeeding mother’s consumption of caffeine may lead to caffeine accumulation and symptoms such as wakefulness and irritability, according to an interview with expert Ruth Lawrence, MD, published in Journal of Caffeine Research, a peer-reviewed journal from Mary Ann Liebert, Inc. Caffeine is found in a wide range of products in addition to coffee, tea, and chocolate, including soft drinks, sports drinks, and some over-the-counter medications.
In a provocative discussion with Dr. Ruth Lawrence, Department of Pediatrics,
University of Rochester School of Medicine and Dentistry, Jack E. James, PhD, Editor-in-Chief of Journal of Caffeine Research, asks a variety of probing questions. Is there a safe level of caffeine intake while breastfeeding? Are there potential long-term effects of caffeine exposure on development and intellect? Can a baby whose mother consumed caffeine during pregnancy experience withdrawal if she then abstains from caffeine while breastfeeding? Dr. Lawrence bases her responses on the scientific and medical evidence related to caffeine exposure in breastfed babies, and distinguishes between what is and what is not well understood in this developing field of study.

“Usually a mother, particularly if she is breastfeeding, is cautioned to limit her caffeine intake,” says Dr. Lawrence, who is Editor-in-Chief of the peer-reviewed journal Breastfeeding Medicine. After giving birth, mothers “should consume all things in moderation and try to avoid the excesses that might really add up to a lot of caffeine.”

Study shows caffeine consumption linked to estrogen changes

Asian women who consumed an average of 200 milligrams or more of caffeine a day—the equivalent of roughly two cups of coffee—had elevated estrogen levels when compared to women who consumed less, according to a study of reproductive age women by researchers at the National Institutes of Health and other institutions.

However, white women who consumed 200 milligrams or more of caffeine a day had slightly lower estrogen levels than women who consumed less. Black women who consumed 200 milligrams or more of caffeine a day were found to have elevated estrogen levels, but this result was not statistically significant.

Total caffeine intake was calculated from any of the following sources: coffee, black tea, green tea, and caffeinated soda.

Findings differed slightly when the source of caffeine was considered singly. Consuming 200 milligrams or more of caffeine from coffee mirrored the findings for overall caffeine consumption, with Asians having elevated estrogen levels, whites having lower estrogen levels, and the results for blacks not statistically significant. However, consumption of more than one cup each day of caffeinated soda or green tea was associated with a higher estrogen level in Asians, whites, and blacks.

The changes in estrogen levels among the women who took part in the study did not appear to affect ovulation. Studies conducted in animals had suggested that caffeine might interfere with ovulation.

The study was published online in the American Journal of Clinical Nutrition.

“The results indicate that caffeine consumption among women of child-bearing age influences estrogen levels,” said Enrique Schisterman, Ph.D., of the Division of Epidemiology, Statistics and Prevention Research at the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), the NIH institute where some of the research was conducted. “Short term, these variations in estrogen levels among different groups do not appear to have any pronounced effects. We know that variations in estrogen level are associated with such disorders as endometriosis, osteoporosis, and endometrial, breast, and ovarian cancers. Because long term caffeine consumption has the potential to influence estrogen levels over a long period of time, it makes sense to take caffeine consumption into account when designing studies to understand these disorders.”

The study authors noted that 89 percent of U.S. women from 18-34 years of age consume the caffeine equivalent of 1.5 to two cups of coffee a day.

The study’s first author was Karen C. Schliep, Ph. D., M.S.P.H., from the University of Utah, Salt Lake City, who conducted the study during a research appointment at NICHD. Dr. Schliep undertook the research with Dr. Schisterman and colleagues at the University of Utah, the NICHD and the State University of New York at Buffalo.

More than 250 women from 18 to 44 years old participated in the study between 2005 and 2007. On average, they consumed 90 milligrams of caffeine a day, approximately equivalent to one cup of caffeinated coffee.

Most of the participants in the study reported to the study clinic one to three times a week for two menstrual cycles. Their visits were scheduled to correspond with specific stages of the menstrual cycle. At the visits, the women reported what they had eaten in the last 24 hours and answered questions about their exercise, sleep, smoking and other aspects of their lifestyle and reproductive hormone levels were measured in blood. The study authors noted that collection of these details during multiple time points across two menstrual cycles produced more precise information about the link between caffeine and hormones than was possible in earlier studies. The researchers also noted that the study participants were more racially diverse than those who took part in previous studies.

Topical Caffeine and Skin Cancer Prevention

There might be a time when instead of just drinking that morning cup of coffee you lather it on your skin as a way of preventing harmful sun damage or skin cancer.

A new Rutgers study strengthens the theory that caffeine guards against certain skin cancers at the molecular level by inhibiting a protein enzyme in the skin, known as ATR. Scientists believe that based on what they have learned studying mice, caffeine applied directly to the skin might help prevent damaging UV light from causing skin cancer.

Prior research indicated that mice that were fed caffeinated water and exposed to lamps that generated UVB radiation that damaged the DNA in their skin cells were able to kill off a greater percentage of their badly damaged cells and reduce the risk of cells becoming cancerous.

“Although it is known that coffee drinking is associated with a decreased risk of non-melanoma skin cancer, there now needs to be studies to determine whether topical caffeine inhibits sunlight-induced skin cancer,” said Allan Conney, director of the Susan Lehman Cullman Laboratory for Cancer Research.

In this newly-published study, instead of inhibiting ATR with caffeinated water, Rutgers researchers, in collaboration with researchers from the University of Washington, genetically modified and diminished ATR in one group of mice. The results: the genetically modified mice developed tumors more slowly than the unmodified mice, had 69 percent fewer tumors than regular mice and developed four times fewer invasive tumors.

The study also found, however, that when both groups of mice were exposed to chronic ultraviolet rays for an extended period of time, tumor development occurred in both the genetically modified and regular mice. What this seems to indicate, says Conney, is that inhibiting the ATR enzyme works best at the pre-cancerous stage before UV-induced skin cancers are fully developed.

According to the National Cancer Institute, sunlight-induced skin cancer is the most prevalent cancer in the United States with more than 1 million new cases each year. Although multiple human epidemiologic studies link caffeinated beverage intake with significant decreases in several different types of cancer, including skin cancer, just how and why coffee protects against the disease is unknown.

“Caffeine might become a weapon in prevention because it inhibits ATR and also acts ad as a sunscreen and directly absorbs damaging UV light,” said Conney.


Caffeine promotes drink flavor preference in adolescents

07/12/11, Clearwater Beach, FL.  Research to be presented at the upcoming annual meeting of the Society for the Study of Ingestive Behavior (SSIB), the foremost society for research into all aspects of eating and drinking behavior, indicates that caffeine added to sugar-sweetened, carbonated beverages teaches adolescents to prefer those beverages.  Researchers found that the amount of caffeine added to an unfamiliar beverage was correlated with how much teenagers liked that beverage.  

“Soda manufacturers claim that caffeine is added to their products to enhance flavor. However, the majority of people cannot taste the difference between caffeinated and non-caffeinated soda.  This led us to suspect that caffeine may be added to beverages for other reasons,” said senior author Jennifer Temple, Ph.D.  ”We hypothesized that adolescents who repeatedly consume a new and unfamiliar drink that contains caffeine would like that beverage more over time, but that adolescents who drank an unfamiliar beverage without caffeine would show no change in their preference.”

To test this theory, adolescents aged 12-17 visited the laboratory multiple times.  During each visit, they sampled an unfamiliar soda drink and rated their liking or preference for that beverage. The sodas contained varying amounts of caffeine, and the caffeinated or non-caffeinated versions were varied across participants.  Over repeated testing days, participants increased their liking of the soda with the highest levels of caffeine, whereas there was no change in preference for sodas with low or no caffeine.

These results are consistent with prior evidence that teens prefer sodas that contain caffeine compared to those that do not, and newly demonstrate that this preference emerges as a learned behavior.

Caffeine & Diabetes -A complicated relationship

New Rochelle, NY, April 7, 2011—A growing body of research suggests that caffeine disrupts glucose metabolism and may contribute to the development and poor control of type 2 diabetes, a major public health problem. A review article in the inaugural issue of Journal of Caffeine Research: The International Multidisciplinary Journal of Caffeine Science, a quarterly peer-reviewed journal from Mary Ann Liebert, Inc. publishers, examines the latest evidence, contradicting earlier studies suggesting a protective effect of caffeine. The entire issue is available free online.

James Lane, PhD, Duke University, describes numerous studies that have demonstrated caffeine’s potential for increasing insulin resistance (impaired glucose tolerance) in adults that do not have diabetes, an effect that could make susceptible individuals more likely to develop the disease. In adults with type 2 diabetes, studies have shown that the increase in blood glucose levels that occurs after they eat carbohydrates is exaggerated if they also consume a caffeinated beverage such as coffee. This effect could contribute to higher glucose levels in people with diabetes and could compromise treatment aimed at controlling their blood glucose.

“More than 220 million people worldwide have diabetes, says Editor-in-Chief Jack E. James, PhD, School of Psychology, National University of Ireland, Galway, Ireland. “The links that have been revealed between diabetes and the consumption of caffeine beverages (especially coffee) are of monumental importance when it is acknowledged that more than 80% of the world’s population consumes caffeine daily.
